The ‘date of first diagnosis’ is the first time you have met with the patient and have a diagnosis that indicates the surgical procedure you are about to enter.

In some instances, you may not have a diagnosis until the day of surgery if it is a diagnostic or emergency procedure. And for some other cases, patients may have had a diagnosis months ago but by a different doctor or surgeon, and they may have been referred to you.

In this circumstance you may not know that original date of diagnosis and we suggest entering the date of diagnosis as you the first time you met with them.
